Step 2 - Print & sign the form(s)
Step 3 - Scan the signed form to your computer
Step 4 - Email the signed form to [email protected]
Option 2
Step 1 - Copy the following sentence:
- "I (your name) - request the director, Lisa Trevino Rodriguez, for GRACE Transition Education Services to electronically sign the following GRACE forms for me: Consent to Disclose Information, Consent to Transport, Receipt of Policies and Procedures, TWC Consent for Disclosure, GRACE Orientation Checklist, GRACE Expectations for Customers."
Step 2 - If you're a student over 18 years of age, email that sentence to [email protected].
Step 3 - For students 18 years of age, parents will also need to send a separate email with the request. The student AND parent must each email a request for electronic signatures.
Step 3 - Scan the signed form to your computer
Step 4 - Email the signed form to [email protected]
Option 2
Step 1 - Copy the following sentence:
